Every list takes per_page (1 to 200, 100 by default) and either page (from 1) or cursor. If both are sent, page wins.

A first call returns in meta: count, next_cursor, page, per_page, total, total_pages, next_page and prev_page (null at the ends).

To walk a long list, pass meta.next_cursor back as cursor until it is null. A cursor call returns only count and next_cursor, which keeps every page fast.

An empty list is a 200 with meta.count 0, never a 404.
